Classification of Dermal Fillers

Dermal fillers are a collagen material made of synthetic or natural substances. It is used for injection in the dermis for the purpose of augmenting soft tissues. Dermal fillers have had a significant rise in their application for aesthetic reasons, namely to help correct skin conditions which develop as a result of ageing such as creases, folds and wrinkles. A broader classification tends to put dermal fillers under permanent or non-permanent depending on their effect. The permanent fillers are also called non-resorbable dermal fillers, while the non-permanent fillers are called resorbable dermal fillers. As the names suggest the permanent dermal fillers can have a long lasting effect. On the other hand the non-permanent dermal fillers have a shorter effect and will require replenishment once they have worn off.

Dermal fillers are supplied in a pre-filled sterile syringe for injection. They are manufactured in different thicknesses; some are more gel-like and others are more liquid. The thicker ones are used to restore the deeper folds or wrinkles such as those between the nose and corner of the mouth. The thinner ones are used more superficially for fine lines such as crow's feet.

Semi-Permanent Dermal Fillers

To achieve a longer visible filling effect, slowly resorbable substances such as polylactic acid (PLA), calcium hydroxylapatite (CaHA), hydroxyethylmethacrylate (HEMA) and dextran (DEAE) have been used. These substances slowly biodegrade and typically persist for three to five years in the tissues. In some cases, multiple injections may initially be required to get the desired effect. Over time, this may need to be repeated because the visible effect deteriorates as the filler breaks down. Dr. Emre Gönen,MD doesn't prefer using semi-permanent fillers for lips.

Permanent Dermal Fillers

To address the issue of materials being resorbed over time, non-biodegradable materials have emerged as dermal filler materials. Liquid injectable silicone (LIS), certain polyacrylamide gels (PAAG), polymethylmethacrylate (PMMA) and

polytetrafluoroethylene (PTFE) are non-resorbable and remain in the tissues permanently. Used correctly, permanent fillers offer certain advantages over non-permanent ones, particularly as they do not require touch-up sessions once the expected result has been achieved. However, permanent dermal fillers are much more technique sensitive than temporary fillers.

Bio-Alcamid™ is a gel polymer comprising exclusively of networks of alkyl-imide groups (approximately 4%) and nonpyrogenic water (approximately 96%). Unlike many other injectable fillers currently available, Bio-Alcamid™ can be used to correct small or large areas, including liposuction deficits. The LIP form is ideal for correcting the lip outline, being imperceptible to the touch.

How long do dermal fillers last?

The length of time that dermal fillers last depends on the type of filler used and the area injected. On average, you would expect the results of a Hyaluronic Acid dermal filler to last around 6-12 months, (Poly-L-Lactic) Polylactic Acid dermal fillers to last around two years, Hydroxylapatite-based dermal fillers appear to last around 9-12 months with most patients getting another treatment after a 12-18 month period.  However some patients need a redo every 6 months. Permanent dermal fillers are not absorbable and therefore they do last in the skin for a lifetime. Thicker dermal fillers tend to last longer than those used for finer lines. As the dermal filler is gradually broken down and the level of volume you gained lessens, you can choose to have the area topped-up to retain the effect. However, if you decide not to, the advantage of non-permanent dermal fillers is that the area will gradually return to how it was before the treatment.

How long does a filler injection procedure take?

Treatment is quick and easy, lasting from fifteen to thirty minutes with immediate results with little to no down time, including minimal swelling and bruising lasting for approximately 24 hours after treatment.

Does dermal fillers treatment need pain relief? Will the procedure hurt?

Generally, pain relief is not necessary when injecting into the skin to correct lines and wrinkles. Some people may experience the treatment as a mild discomfort. A local anaesthetic cream can be applied to the area for a more comfortable treatment experience. The lips are a highly sensitive part of the body and therefore, anaesthesia is usually recommended before lip enhancement. This is achieved most effectively by using a local anaesthetic. Dermal fillers are also available with the local anaesthetic 0.3% lidocaine included for a more comfortable experience. What should I expect following the procedure?

As with any treatments that require injection there may be some discomfort, redness or swelling after treatment. There may also be some localised tenderness. This is normal and will generally disappear in a day or two. You can apply an ice pack for a brief period, especially after lip augmentation procedure.
